More than 20% burns are usually affected by Inhalation Injury, either Thermal or Secondary to Sepsis.
Hence More than 20% Brns require Prophylactic Antibiotics.
Bronchodilators should be restricted because it increases mortality.
Brochoscopy showing inflamed red mucosa is an early sign of inhalation injury.
Early intubation reduces morbidity.
Tracheostomy is reserved for lower respiratory tract or parenchymal injury due to toxic gas inhalation. viz Carbon Monoxide & / or hydrogen cyanide which has an odour of bitter almond,
Carbon monoxide is a colourles, odourless gas and has affinity for haemoglobin forming carboxyhaemoglobin and also acts on mitochondria, while cyanide causes reversible anoxia at cytochrome oxidase level.
the best antidote and management is Hydroxocobalamine 5mg iv.
Due to anoxia patient develops lactic acidosis, with increase in level of lactates in blood.
